Education, clinical judgment and modern orthodontics
One of the most common questions when a patient comes in for orthodontics in Montevideo is: “Will I need to have teeth pulled?”
The responsible answer is not an automatic yes or no. The decision depends on a comprehensive diagnosis by an orthodontic specialist. Modern orthodontics does not extract teeth as a matter of routine, but it does not avoid extractions when they are correctly indicated either.
Why were extractions more common in the past?
For decades, the usual answer to crowding or lack of space was to extract premolars to make room. In many cases that was the right call based on what was known at the time. However, advances in biomechanics and facial diagnosis have changed the way treatment is planned.
Our clinical approach starts with the patient's face. Modern orthodontics is no longer just about “making space”, but about achieving harmony and stability.
When can extractions be avoided?
In cases of mild or moderate crowding, strategies such as these can be used:
- Controlled dentoalveolar expansion
- Distalization of the back teeth
- Efficient biomechanical management
These tools redistribute space without compromising facial aesthetics or bite stability. But every patient has a different anatomy and bone base: not every case is a candidate for treatment without extractions.
When are extractions necessary?
They may be the best option in cases of:
- Severe crowding
- Markedly protruding incisors
- A convex facial profile that requires retraction
- Significant skeletal imbalances
- Periodontal problems
Avoiding extractions when they are indicated can lead to instability, excessive protrusion, relapse and functional problems. Responsible orthodontics is based on clinical diagnosis, not on commercial promises.
Does modern orthodontics eliminate extractions?
No. It widens the range of treatment options and makes it possible to consider alternatives before deciding on an extraction, but it does not rule extractions out when they are needed. The end goal is always stability, correct function, periodontal health, facial harmony and long-lasting results.
Why an orthodontic specialist matters
If you are considering orthodontic treatment in Montevideo, it is essential that the diagnosis is made by a dentist who specializes in orthodontics, with postgraduate training and the corresponding professional license.
